If by "third-world" he means displays characteristics associated with areas colonized by Europeans/Americans, then those are (largely) the results of imperialism and its effects on the colonized society. Pointing to deformities in those political systems as if they were self-generated and foreign to "us Americans" is a perpetuation of this imperialist thinking.

These are just bullet points, but he reveals some of what's behind Trump's dystopian ideology. Digging deeper does implicate the US.
Yeah, the bullet point approach doesn't allow for subtlety.
Because Mbeki's AIDS denialism was based on Anglo-American claims, and herbal cures and other nonsense have a well-established history in all kinds of other places, so holding it up as a "these crazy people in Africa and now we're thinking like them" is a distortion, not a revelation.
